This new role represents a unique opportunity to launch and grow a pediatric rheumatology service that will serve as a center of excellence, meet community needs, and ensure high quality care for pediatric patients with rheumatologic conditions. The clinic is part of a cost-savings initiative that provides free appointments, medications, counseling, and education to patients, while also expanding access to high-quality specialty care. This opportunity will include working alongside a highly collaborative team that includes an Endocrinologist, Rheumatologists, an APRN, and a clinical pharmacist.

Key Responsibilities:

  • Panel Growth: Build and manage a patient panel through inpatient consults and follow-up visits, with support from a multidisciplinary team.
  • Outpatient Care: Deliver compassionate, evidence-based outpatient care to pediatric patients in a high-volume clinical setting.
  • Integrated Collaboration: Partner closely with endocrinologists, pharmacists, and other healthcare professionals to deliver coordinated, multidisciplinary care plans.
  • Clinic Development: Lead the establishment and expansion of pediatric rheumatology services within the Pharmacy Specialty Clinic.
  • Continuity of Care: Support smooth transitions of care for patients aging out of pediatric services into adult rheumatology care within the system.
  • Community Engagement: Promote awareness of pediatric rheumatology services through outreach initiatives and education.
